Provider Affiliation

Provider Changes - Affiliate one or more NPIs to submit electronic claims or receive electronic remittance advice (ERA) through an existing trading partner.

Trading Partner Contacts

The following forms are available to update contact information for existing trading partners:

Update Administrator/Delegate - Update administrator or delegate information for a trading partner.
Update Address - Update the main address, phone, or email for a trading partner.
Update Software Vendor - Update software vendor information for a trading partner.

The following forms are available to update the capabilities of existing trading partners:

Update EDI Transactions – Add/update transactions for a trading partner.
Delete Trading Partner - Delete a trading partner.
ERA Enrollment Form - Enroll ERA.
